New Medical Forms

BSA has issued a new medical form that replaces the Class 1, 2 and 3 forms. Usage of the new form is optional but encouraged in 2009. The new form is mandatory beginning January 1, 2010. For 2-page version, click here.  For 4-page version, click here.

Merit Badge Counselors Registered GAD Scouters can request the current list of GAD counselors by sending a request to Wayne Vincent. (This 47-page list was last updated in September 2008.)
